Review and Evaluation of Treatment Procedures Using Injectable Poly-L-Lactic Acid in the Treatment of Human Immunodeficiency Virus–associated Facial Lipoatrophy

December 2010

Douglas Mest, MD; Gail Humble, MD, Blue Pacific Aesthetic Medical Group, Hermosa Beach, California

Abstract
Injectable poly-L-lactic acid is United States Food and Drug Administration–approved for the restoration and/or correction of the signs of lipoatrophy in people with human immunodeficiency virus and for use in immune-competent people for the correction of nasolabial fold contour deficiencies. Current prescribing information recommends using either a tunneling or threading technique in a grid or cross-hatched pattern or a depot technique (in thin skin areas) to administer multiple passes of injectable poly-L-lactic acid per treatment session via a 26-gauge needle. A review of the literature and the recently published guidelines from the American Society for Dermatologic Surgery provide evidence to support the use of a cross-fanning technique with a 25-gauge needle. In conclusion, the use of a cross-fanning injection technique with a 25-gauge needle to administer injectable poly-L-lactic acid provides a clinically valid, alternate delivery option for the treatment of patients with human immunodeficiency virus-associated lipoatrophy.

Application of Nutrigenomics in Skin Health: Nutraceutical or Cosmeceutical?

November 2010

M.T. Ravi Subbiah, PhD

Dr. Subbiah is from the Department of Internal Medicine, University of Cincinnati Medical Center, Cincinnati, Ohio. Disclosure: Dr. Subbiah reports no relevant conflicts of interest.

Abstract
Nutrigenomics is a growing field related to genetic testing based on the documentation of genetic mutations in an individual, giving us the ability to correct metabolic imbalances (personalized medicine) through specific dietary supplements or nutraceuticals. An off-shoot of nutrigenomics called dermagenetics (testing for selected genetic mutations related to skin health followed by advocating the use of either nutraceuticals or skin creams enriched by cosmeceuticals) is heading toward commercialization at a rapid rate by directly targeting the public. Although this growth represents an opportunity to explore the benefits of genetic advances in skin health, it is essential that the science, product claims, and ethical standards be critically evaluated and clear national guidelines be set in order to protect the consumer. Immunophenotypic Diagnosis of Primary Cutaneous Lymphomas: A Review for the Practicing Dermatologist

October 2010

Peters, MD, The Ohio State University, Division of Dermatopathology, Columbus, Ohio; Matthew J. Zirwas, MD, The Ohio State University, Division of Dermatology, Columbus, Ohio; Henry K. Wong, MD, The Ohio State University, Division of Dermatology, Columbus, Ohio

Disclosure: Drs. Campbell, Peters, and Zirwas have no conflict of interests to declare. Dr. Wong has served as an advisor for EISAI.

Abstract
The application of CD markers to medicine has advanced our understanding of several dermatological diseases, most notably primary cutaneous lymphomas. CD markers are monoclonal antibodies that target cell surface molecules on leukocytes and antigens from other cells. T-cell processes are typically CD3+, CD20-, CD45+ while B-cell processes are typically CD3-, CD20+, and CD45+. Other CD markers are used to further delineate cutaneous lymphomas. Although an imperfect system, CD markers empower dermatologists to synthesize immunophenotyping with clinical findings and formulate an appropriate diagnosis.  (J Clin Aesthet Dermatol. 2010;3(10):21–25.)
